Basic Care Management
Getting care from many different providers or in different health systems is challenging.
L.A. Care wants to make sure members get all medically necessary services, prescription medicines, and behavioral health services.
L.A. Care can help coordinate and manage your health needs, at no cost to you. This help is available even when another program covers the services.
It can be hard to figure out how to meet your health care needs after you leave the hospital or if you get care in different systems.
Here are some ways L.A. Care can help members:
- If you have trouble getting a follow-up appointment or medicines after you are discharged from the hospital, L.A. Care can help you.
- If you need help getting to an in-person appointment, L.A. Care can help you get free transportation.

Complex Care Management
Members with more complex health needs may qualify for extra services focused on care coordination.
L.A. Care offers Complex Care Management (CCM) services to members with:
- Hard to manage health conditions, or
- Health issues that are getting worse, or
- Trouble getting caregiving at home, or
- Multiple emergency room or hospital visits in the past 6 months.
Members who are enrolled in CCM and Enhanced Care Management have an Assigned Care Manager at L.A. Care who can help with not only basic care management described above, but also an expanded set of transitional care supports that are available if you are discharged from a hospital, skilled nursing facility, psychiatric hospital, or residential treatment.
