Community Supports Services

You may get supports under your Individualized Care Plan.

Community Supports are medically appropriate and cost-effective alternative services or settings to those covered under the Medi-Cal State Plan. These services are optional for members.
If you qualify, these services might help you live more independently. They do not replace benefits you already get under Medi-Cal.
Community Supports may include help finding or keeping housing, medically-tailored meals, or other help for you or your caregiver.

There is no cost to members for Community Supports services.

The following are Community Supports offered by L.A. Care:

  • Housing Transition Navigation Services & Housing Tenancy and Sustaining Services
    • Includes services to help members find housing and maintain safe and stable tenancy.
  • Meals/Medically Tailored Meals
    • Home-delivered meal programs that aims to improve health outcomes and address barriers associated with food insecurity.
  • Recuperative Care (Medical Respite)
    • Short-term integrated and clinical care for members who no longer require hospitalization but still need to health from an injury or illness (including behavioral health conditions).
  • Housing Deposits
    • Assists with identifying, coordinating, securing, or funding one-time services and modifications necessary to enable a person to establish a basic household, it does not constitute room and board.
  • Sobering Centers
    • Alternative destinations for members who are found to be publicly intoxicated and would otherwise be transported to the emergency department or jail.
  • Personal Care and Homemaker Services
    • Supports members who need assistance with daily activities, such as bathing, getting dressed, personal hygiene, cooking and eating.
  • Respite Services
    • Short-term services provided to caregivers of those who require occasional temporary supervision to give relief to the caregiver.
  • Environmental Accessibility Adaptations (Home Modifications)
    • Physical adaptations to a home to ensure the health and safety of the individual, or enable the member to function with greater independence in the home.
  • Asthma Remediation
    • Provides physical modifications to a home environment that are necessary to ensure the health, welfare, and safety of the member, or enable the member to function in the home and without which acute asthma episodes could result in the need for emergency services and hospitalization.
  • Nursing Facility Transition/Diversion to Assisted Living Facilities
    • Helps members live in the community by facilitating transitions from a nursing facility back into a home-like, community setting, or preventing nursing facility admissions for those with imminent need.
  • Community Transition Services/Nursing Facility Transition to a Home
    • Assists members to live in the community to avoid further institutionalization by providing non-recurring set-up expenses for members transitioning from a licensed facility to a living arrangement in a private residence.
  • Short-Term Hospitalization Housing
    • Provides those who do not have a residence, and who have high medical or behavioral health needs, the opportunity to continue their medical, psychiatric, or substance use recovery immediately after exiting an inpatient institutional setting.
  • Day Habilitation Programs
    • Provides services in or out of a person’s home to assist them in acquiring, retaining, and improving self-help. Socialization, and adaptive skills necessary to reside successfully in the community.
