L.A. Care Commits $500,000 to a Project That Will Improve Care Coordination and Health Outcomes

The Funding Will Modernize a Health Information Exchange Platform

LOS ANGELES – The collection and timely delivery of accurate healthcare information between patient and provider could be the difference between life and death. The Los Angeles Network for Enhanced Services (LANES) has been an important health information exchange for hospitals and clinics in Los Angeles County since launching in 2010. In 2018, L.A. Care became the first health plan to partner with LANES to ensure proper care coordination for its members. Since then, LANES has seen tremendous growth and has outgrown the capacity of its current technology platform, so L.A. Care is investing $500,000 in a project that will modernize the health information exchange and increase its capacity.

LANES has been a valuable partner, and we want to ensure it can continue to execute timely reports and help reduce costly errors in medical decisions that can result in adverse drug effects or duplicate testing,” said John Baackes, L.A. Care CEO. “The project will support the California Advancing and Innovating Medi-Cal (CalAIM) by connecting and facilitating care coordination across entities providing clinical and behavioral care, and social services.”

LANES currently serves three health plans, 53 healthcare organizations, 33 Federally Qualified Health Centers and community clinics, and 17 hospitals.

Our scalable platform expands the analytics and population health capability to strengthen care delivery for a more equitable, coordinated person-center approach,” said Ali Modaressi, LANES CEO. “The L.A. Care grant will help LANES accelerate Cloud technology with FHIR capabilities, a technology that will help us continue our mission of connecting hundreds of healthcare organizations serving the safety net population and will improve health outcomes.“

LANES is designed to provide a single point connection with every provider in its network, avoiding a myriad of unnecessary interfaces when securing patient data from multiple providers. Ensuring the accurate and timely transfer of health information between providers will lead to positive health outcomes for L.A. Care members and others in their communities.
