Short-Term Post-Hospitalization Housing can help you if you do not have a residence and have high health needs. It gives you a safe place to recover after you leave a facility.
Examples of facilities:
- Inpatient hospitals (either acute or psychiatric or Chemical Dependency and Recovery hospital);
- Residential substance use disorder treatment or recovery facility;
- Residential mental health treatment facility;
- Correctional facility;
- Nursing facility; or
- Recuperative care
You can get support to help avoid returning to a facility. You may also get help with learning (or relearning) how to do daily tasks and get needed medical care, treatment, and case management.
The program also helps connect you with Housing Navigation services to find housing. Someone can help you plan for when you leave the program. They can talk to you about what kind of home setting you want and any problems you may have to stay housed.
Eligibility
To join the program, you must be an active member with Medi-Cal or D-SNP. You must also have the following:
- You must be exiting a facility;
- You must be homeless or at risk of homelessness.
Also, you must have one of the following:
- One or more serious chronic conditions;
- Serious mental illness;
- Are at risk of being put in a facility or overdose;
- Are receiving Enhanced Care Management; or
- Are a Transition-Age Youth with unstable housing.
Other restrictions may apply.
