Focus on Mothers and Babies

L.A. Care is committed to the health and well-being of mothers and their babies.  That's why we offer the "Healthy Pregnancy", "Healthy Mom", and "Healthy Baby" programs for your L.A. Care patients.  

These three educational outreach programs work in conjunction with one another to improve pregnancy outcomes, ensure appropriate postpartum care, and promote timely well child visits and immunizations.  

Through the "Healthy Pregnancy" program, members are mailed trimester-specific, culturally appropriate educational materials on planning a healthy pregnancy, nutrition, care after childbirth, breastfeeding and a Newborn Referral Form (MC 330) — a state Medi-Cal enrollment form.  

Members identified in their first trimester of pregnancy, or within 42 days of enrollment, receive a live agent call and are offered assistance with scheduling a prenatal visit. Members receive a onesie for their newborn as a thank you once the prenatal visit is confirmed.

The "Healthy Mom" postpartum program offers assistance scheduling the postpartum appointment, in addition to conducting in-person visits in targeted regions to women with limited access to care. Members may also receive transportation assistance and interpreting services to reduce barriers, if needed. Members who complete the postpartum visit are eligible to receive a $40 gift card as a thank you.

The "Healthy Baby" program is a member outreach program aimed at improving immunization rates among members under the age of 24 months. Program components include targeted educational mailings to parents/guardians and an interactive voice response (IVR) mailer reminder call.
