Community Supports: Medically-Tailored Meals
Medically Tailored Meals (MTM) is a therapeutic nutrition intervention aimed at improving health outcomes and reducing hospital readmission rates.
Designed by Registered Dietitian Nutritionists (RDN), home-delivered meals support referring providers in treating chronic illness, targeting specific nutritional needs of the patient while reducing barriers associated with food insecurity.
Eligible members receive:
- Home-delivered, healthy meals tailored to their health needs.
- At least 2 meals per day for up to 12 weeks, with a possible extension up to 26 weeks.
- Nutrition education from a health educator or Registered Dietitian.
To qualify, a member must be living with advanced congestive heart failure, chronic kidney disease (stages 3-4), or diabetes (with multiple or high doses of medication). The member must also have a history of hospital inpatient stays or emergency department visits during the past 12 months.

Please note that a referral does not guarantee participation. Members that are institutionalized, or do not meet suitability criteria as determined by a Registered Dietitian Nutritionist may be excluded. For more information, please contact L.A.
