nurse on phoneOur concurrent review nurses now manage all inpatient facility discharge authorizations and coordinate difficult placement assistance referrals. Providers with L.A. Care members in acute inpatient hospitals should call 877-431-2273, select option 3, and then option 2 to be transferred to the Utilization Management (UM) Call Center.

The UM Call Center will promptly direct all provider calls to the inpatient nurse overseeing the acute inpatient admission. The inpatient nurse is responsible for providing the necessary inpatient discharge authorizations directly to the provider, ensuring a streamlined process for both discharge and placement assistance.

This system allows for efficient management of discharge procedures and facilitates coordination for challenging placements. By centralizing communication through the UM Call Center and involving inpatient nurses in direct authorizations, L.A. Care ensures timely and accurate handling of inpatient care transitions.

This approach helps maintain continuity of care and addresses the complex needs of patients requiring specialized discharge planning and placement assistance.
