L.A. Care continues to close care gaps and improve patient care with partners such as DocGo.
DocGo, an in-home health service partner, will offer all eligible L.A. Care Covered and L.A. Care Medicare Plus (HMO D-SNP) patients in-home and virtual health care.
Benefits and Services Include:
- Post-Inpatient Stay follow-up
- Post-Emergency Department follow-up
- Clinical Services
- Charting and Follow-Up
- Well Child Visits
- Child Vaccinations
- Evaluations for patients with diabetes
- Regional Outreach
- House Call or Telehealth Appointment Scheduling
- And other health services at our members’ doorsteps.
L.A. Care providers will maintain full incentive payments of Pay for Performance and IPA Performance for their Participating Provider Group Value Initiative, even when their patients are seen by a DocGo provider.
DocGo visits began January 2025 and patients may now schedule their appointments.
Attendees such as parents or guardians of members will be contacted directly by DocGo to schedule an appointment.
- Patients Medical Charts:
- L.A. Care providers will receive them via e-fax after the visit.
- Independent Practice (or Physician) Associations (IPAs) will receive them through the listed primary care physician number linking to an individual doctor, clinic or larger entity.
