Keeping Minds Sharp with the Annual Cognitive Health Assessments (ACHA)

The Annual Cognitive Health Assessment (ACHA) aims to ensure timely detection and early intervention for Alzheimer’s disease or related dementias. L.A. Care and DHCS require providers to conduct an annual ACHA for all Medi-Cal members 65 years and older who do not have Medicare coverage.

Provider Requirements:

  • Complete the DHCS Dementia Care Aware cognitive health assessment training
  • Conduct an ACHA on eligible Medi-Cal members 65 years and older assigned to you.
  • Conduct appropriate follow-up services based on ACHA findings.
  • Document all the following in the Member’s medical record and have records available upon request.
  • Use CPT code 1494F to submit proof of rendered services.

Providers are encouraged to utilize resources available, including the Dementia Care Aware Warmline available to health care providers at 1-800-933-1789, Monday to Friday, 9 am – 5 pm for education and decision-making consults related to dementia screening, assessment, diagnosis, management, and care planning.
