One such appointment is the Annual Cognitive Health Appointment (ACHA) to ensure timely detection and early intervention for Alzheimer’s disease or related dementias. L.A. Care and DHCS require providers to conduct an annual ACHA for all Medi-Cal members 65 years and older who do not have Medicare coverage, as outlined in APL 22-025.  

Provider Requirements:

  • Complete the DHCS Dementia Care Aware cognitive health assessment training
  • Conduct an ACHA on eligible Medi-Cal Members 65 years and older assigned to them
  • Conduct appropriate follow-up services based on the ACHA findings
  • Document all of the following in the Member’s medical record and have such records available upon request
  • Use CPT code 1494F to submit proof of rendering services

Reminder, providers using CPT code 1494F can be used when a CHA-trained provider performs the screening for a Fee for service Medi-cal only beneficiaries over 65.

Providers are encouraged to utilize resources available, including the Dementia Care Aware Warmline available to health care providers at (800) 933-1789, Monday to Friday, 9 am – 5 pm for education and decision-making consults related to dementia screening, assessment, diagnosis, management, and care planning.
