At L.A. Care, we are committed to partnering with providers to deliver the best possible care for our patients. We are thrilled to announce the launch of a new at-home diagnostics program to close care gaps for patients.
By offering easy-to-use, at-home sample collection kits, we hope to empower patients, including vulnerable populations, to take charge of their health from the convenience and comfort of their home. On December 1, 2023, we shipped kits to patients with FIT, HbA1c, and/or KED care gaps. All kits will be accompanied by personalized outreach and some financial incentives to encourage patients to send their samples back to the lab.
The program, which is designed in partnership with Walgreens and ixlayer focuses on inclusivity and patient-centricity to effectively reach patients where they are. Patients will have the option of using a secure digital portal to view instructions and results, or complete the entire process without needing to go online. Everything that a patient needs to complete the test will be included in the kit, including a prepaid return shipping envelope.
Results will be provided to the patients and to their physician. Physicians can expect to receive results within two weeks of a patient collecting their sample. This allows you to gain critical insights into your patients' health conditions, enabling timely interventions and personalized treatment plans - all with the ultimate goal of improving patient outcomes.
