Also known as Homeless and Housing Support Services (HHSS), this includes services to help members who are homeless find housing and maintain safe and stable tenancy. HHSS provides two services to eligible members: housing navigation and tenancy sustaining services.
- Housing Navigation — services to help homeless members find housing.
- Tenancy Sustaining Services — services to help formerly homeless members maintain safe and stable tenancy once housing is secured.
- Please note that HHSS does not offer housing subsidies or housing vouchers.

Restrictions and Limitations
- HHSS has been identified as not reasonable and necessary, as documented in Member’s Individualized Housing Support Plan (IHSP). Member is enrolled in a duplicative housing navigation or tenancy sustaining services program.
- Member declines services.
- Member has previously received Housing Tenancy and Sustaining Services Community Supports (limit of a single duration in the individual’s lifetime; services may be approved one additional time with documentation as to what conditions have changed to demonstrate why services would be more successful on the second attempt).
- Members enrolled with plan partners (Anthem Blue Cross, Blue Shield of California Promise Health Plan) are not eligible for L.A. Care's HHSS program.

Provider Referral Process
- HHSS provider identifies potentially eligible member for HHSS.
- HHSS provider submits HHSS referral via secure fax or email.
- L.A. Care reviews HHSS referral and approves, denies, or requests more information.
