L.A. Care provides Community Supports (CS), medically appropriate and cost-effective services and settings to address Members’ health-related social needs. Community Supports are intended to help Members lead healthier lives and avoid higher, more costly levels of care. The Quick Reference Guide is an easy-to-use resource for Providers that summarizes the CS offered, how to refer a Member, and contact information for each service.
Examples of services include assistance with finding or securing housing including one-time help with security deposits, healthy meals or grocery boxes delivered to the Member’s home, and help with daily tasks like bathing, dressing, or meal preparation, and more.
