Day Habilitation

Day Habilitation programs can be given in or out of your home. The program may help you learn or grow skills like self-care, working with others, and doing things on your own.

More skills you may learn to help you live better in your community are:

  • How to use public transportation;
  • How to deal with and solve problems;
  • How to be a part of the community; 
  • How to build and grow relationships;
  • How to cook, clean, and other daily living skills;
  • How to shop and handle money;
  • How to access local support such as police, fire, and other aids. 

Other help you may receive is:

  • Finding and moving into a home;
  • Finding and choosing housemates;
  • Finding home items and furniture;
  • Taking care of landlord issues;
  • Taking care of finances (money issues);
  • Finding, teaching, and managing helpers that help you with daily tasks;
  • Talking to government workers and offices;
  • Protecting your rights;
  • Building a support group;
  • Connecting you to other health care and social services you may need that are available through L.A. Care, such as other Community Supports or Enhanced Care Management;    
  • Helping you find other housing aid if you do not qualify for Housing Transition/Navigation Services; 
  • Help with wages and programs like General Assistance/General Relief and SSI; and 
  • Working with your care team to get other services you may need such as healthcare, mental health support, and to help with drug use. 

Eligibility: 

To join the program, you must be an active member with Medi-Cal. You must also have one of the following:

  • You meet the definition of being homeless; OR
  • You were homeless but found a home in the last 2 years; OR
  • You meet the definition of being at risk of losing your home; OR 
  • You were at risk of having to stay in a place like a hospital or nursing facility.
