Environmental Accessibility Adaptations (Home Modifications)

Environmental Accessibility Adaptations also known as Home Modifications are changes made to the home to:

  • help you live safely and stay healthy;
  • be more independent; and
  • help avoid going to a nursing home.

Examples of Environmental Accessibility Adaptions include:

  • Ramps and grab-bars to help you go in and out of the home;
  • Wider doorways if you use a wheelchair;
  • Stair lifts;
  • Making a bathroom and shower wheelchair easy to access; 
  • Setting up special electric and plumbing systems for medical equipment; and
  • Personal Emergency Response Systems (PERS)

 

Eligibility:

To join the program, you must be an active member with Medi-Cal. You must also:

  • Be at risk of going to a nursing home, and
  • Be at risk for being hospitalized.

You will need to work with your case manager or doctor to get a written request to get these services. Click HERE for the link to the order form. To receive services, you will also need approval from the homeowner (if you do not own the home). The EAA provider will give you a form for the homeowner to sign.
