What is Cal MediConnect?

Cal MediConnect is an all-in-one health plan to serve those who are eligible for both Medicare and Medi-Cal. It covers medical, prescription medicines and Independent Living Services.

Do you need help staying safe and healthy at home?

If you need support to help you live independently in your own home, members may be eligible for the Independent Living Services below.

  • Meal Delivery
  • Assistance with daily activities: cooking, cleaning, and personal care
  • Home Safety Modification: shower grab bars, railings, wheelchair ramp installation
  • Medical Alert Device
