Cal MediConnect ID Card
You should have already gotten a L.A. Care Cal MediConnect Plan Member ID Card, information about how to access a Provider and Pharmacy Directory, and a List of Covered Drugs.
Under our plan, you will have one card for your Medicare and Medi-Cal services, including long-term services and supports, certain behavioral health services, and prescriptions. You must show this card when you get any services or prescriptions.
Getting Your L.A. Care ID Card
As a member of Cal MediConnect, you will receive a member ID card when you first join our health plan and whenever we make changes to the appearance or content of the member ID card.
If you lost your ID card, changed your primary care provider, or changed medical groups, you can
- Get a new ID card mailed to you. Call Member Services at 1-888-522-1298 (TTY: 711) 24 hours a day, 7 days a week, including holidays.
- Print an ID card from your L.A. Care Connect member account. You must have or create an account.
