Los Angeles – L.A. Care is proud to be one of the top three performers among 29 health care organizations who received a Transforming Clinical Practice Initiative (TCPI) award from the Centers for Medicare and Medicaid Services (CMS). 

TCPI is a four-year program designed to equip more than 140,000 doctors and clinicians nationwide with tools and support needed to improve quality of care, increase patients’ access to information and spend dollars more wisely.  

Our TCPI team is definitely helping to transform practices in Los Angeles County. As they help medical professionals use data to improve quality, they are ultimately improving the health outcomes for our members, and that is the primary goal.

L.A. Care received a $16 million grant in 2015 to help 3,200 L.A. County primary care and specialty clinicians serving more than one million patients improve care for diabetes and/or depression and transform their practices. Just two-and-a-half years later, the L.A. Care program is already meeting target points for several diabetes and depression related clinical measures. 

For example, the L.A. Care program has seen:

  • an 82 percent improvement in suicide risk assessment in children;
  • a 54 percent improvement in suicide risk assessment in adults;
  • a 17 percent improvement in BMI screenings with follow up; and
  • an 11 percent improvement in utilization of a screener that monitors symptoms of depression. 

In all, L.A. Care has reached 62 percent progress on its commitment.

The headlines are filled with stories about health care costs skyrocketing, but the TCPI program has produced tens of millions of dollars in cost savings for the 3,200 clinicians in the L.A. Care program through fewer hospitalizations, better care coordination and management, and more patient engagement. 

CMS, which has committed $685 million to TCPI, is gratified by L.A. Care’s success in supporting clinician practices, while increasing patients’ access to information.
