Cal MediConnect ID Card

You should have already gotten a L.A. Care Cal MediConnect Plan Member ID Card, information about how to access a Provider and Pharmacy Directory, and a List of Covered Drugs.

Under our plan, you will have one card for your Medicare and Medi-Cal services, including long-term services and supports, certain behavioral health services, and prescriptions. You must show this card when you get any services or prescriptions.
 

Getting Your L.A. Care ID Card

As a member of Cal MediConnect, you will receive a member ID card when you first join our health plan and whenever we make changes to the appearance or content of the member ID card.
 

If you lost your ID card, changed your primary care provider, or changed medical groups, you can

  1. Get a new ID card mailed to you. Call Member Services at 1-888-522-1298 (TTY: 711) 24 hours a day, 7 days a week, including holidays.
  2. Print an ID card from your L.A. Care Connect member account. You must have or create an account.
     

Understanding Your ID Card

Your Member ID Card contains important information. Below is an explanation of what you will find displayed on your Member ID Card.

Cal MediConnect Member ID Card Front

Cal MediConnect Plan: This Plan Name indicates that you are enrolled in the L.A. Care Cal MediConnect (Medicare-Medicaid Plan) health insurance product.

L.A. Care Health Plan: This logo indicates that the L.A. Care Cal MediConnect health insurance product is part of L.A Care.

Member Name: Your name will be displayed on the Member ID Card. Only the person indicated on the Member ID Card may use this card to obtain services.

Member ID:  This number is unique to you and can be used to create a member profile on L.A. Care Cal MediConnect’s webpage. There you can find out other information such as eligibility status and claims history.

Health Plan (80840): 7477556592: This is a unique plan identification number. Your providers may use this number.

Effective Date: The date your health coverage begins.

PCP Name: The primary care physician you selected or was assigned to you. You should visit your primary care physician for all health care needs. Your primary care physician will be able to refer you to a specialist depending on your medical needs.

Medical Group: The medical group you are assigned to. This is the medical group your primary care physician is associated with. When you need to see a specialist, your primary care physician will refer you to a doctor within your assigned medical group.

Rx Information: Your pharmacy/pharmacist needs this information.

Medicare Rx: This logo is a symbol of your Medicare drug coverage.

Member Services: This is the number you can contact regarding general information such as provider availability, drug coverage, and the health benefits covered under your plan.

Behavior Health Services: This is the number you can contact regarding behavior health services such as behavioral, psychological and emotional health.

Dental Services: This is the number you can contact if you need help finding a dentist in our network.

24-Hour Nurse Advice Line: This is the number you can contact if you have questions regarding a non-emergency medical situation or concern. A nurse will be available 24 hours a day, 7 days a week to answer your questions and offer advice.

PCP Phone Number: This is the number you can contact to reach you primary care provider. Call this number to make appointments with your doctor.
